Initial Nursing Assessment
A full nursing assessment at home covering health history, medicines, skin, mobility and home safety, with a written care plan.
Wound Assessment & Dressing
A full wound assessment and dressing at home, with a written wound care plan. Time can vary with the size and type of wound.
Medication Management
Help taking medicines safely at home: Webster pack checks, a review of what you take, and prescribed injections such as insulin.
Continence Assessment & Report
A bladder and bowel assessment at home with a written report and care and product recommendations, suitable for NDIS plans.
Catheter Care (IDC & SPC)
Planned catheter changes and catheter care at home for indwelling urethral (IDC) and suprapubic (SPC) catheters.
Post-Hospital Discharge Visit
A nurse visit in the first days after hospital to check recovery, medicines and wounds, so you can stay safely at home.
Health Check & Observations
Regular nurse visits to monitor vital signs, weight, skin and wellbeing, with changes reported to your GP.
Care Planning & GP Liaison
Nurse-led coordination with your GP, specialists, allied health and support workers, so your care plan is followed.
Prices are private weekday rates and GST-free. NDIS participants pay the NDIS price limit, shown on each service page and in the fees below. Travel is extra, and dressings, catheters and other supplies are charged at cost. We confirm everything before your visit.

Equipment, products & referrals
Everything you need to stay well at home
After an assessment or hospital stay, your nurse may recommend practical help alongside your nursing care. We explain why each item is suggested and help you arrange it.
Recommendations are made within the Registered Nurse's scope of practice. Complex equipment and home modifications are referred to an occupational therapist. Products are charged at cost, and many items can be funded through NDIS or Support at Home.
- Daily living equipmentSimple aids for safety and comfort, such as a shower stool, over-bed table or bed cradle.
- Skin & pressure careSkin integrity assessment and products such as barrier creams, emollients and pressure-relief aids.
- Continence productsAdvice and fitting for pads, urisheaths and other continence products that suit you.
- Allied health referralsReferrals to occupational therapy, physiotherapy, podiatry, dietitians and others, shared with your GP.
How it works
From request to first visit
- Send a requestChoose a service and complete the short form. Families, GPs and coordinators can request on someone's behalf.
- We call youWithin 1 business day we confirm needs, visit time, travel and who pays.
- Agreement & consentYou receive a short service agreement to sign before care begins.
- Your nurse visitsCare is recorded at each visit, and your GP or provider is updated with your consent.
For GPs, practice nurses & coordinators
Refer a patient for nursing at home
We work alongside general practice to keep patients well at home between appointments. Referrals are accepted from GPs, practice nurses, specialists, hospitals, support coordinators and families.
- Contact with the patient within 1 business day of referral
- Assessment summary to the referring GP within 2 business days
- Wound progress reports and prompt escalation of any concerns
- Works with your existing medication orders and care plans

No GP referral is needed for private clients, but we welcome one. Please include current medications and any wound or medication orders.

Vesta Nursing is a Sydney home nursing practice led by an experienced Registered Nurse with a background in community and aged care nursing, including wound care and support for people living at home.
We provide person-centred, evidence-based care in your home or supported accommodation, working with your GP, allied health team, family and support coordinators. Each visit is planned around your goals and consent, delivered within the nurse's scope of practice, and recorded clearly.
